Insurance Information:

• For Medicare:

  I can bill for Original Medicare and Medicare Advantage PPO. I am unable to bill for a Medicare Advantage HMO, but you may pay cash if you are interested in treatment. Please be aware that currently most nutrition services are not covered by Medicare.

• For non-Medicare insurances:

  I do not directly bill to insurance, but I can provide you with a superbill to submit to your insurance. You will need to be aware of what your insurance requires from you to submit a superbill, as it will vary by insurance company. Often a physician prescription is needed but sometimes if there is either an FSA or HSA you may not need one. It is your responsibility to know your specific plan requirements

• I am currently researching insurance options for nutrition, but this will vary by state. I am located in California
